This provider offers services through the Commonwealth Home Support Program.
Midlands Planned Activity Group program provides weekday social support activities for adults in the community. Monday to Friday groups for up to 25 participants. On-site activities and outings to local community venues. Limited small bus transport available to assist access to the program. Fee schedule available on request. Currently $20.50 per day for attendance and meal for CHSP eligible participants. Those in receipt of funding through Support at Home and NDIS may participate at an alternative fee schedule if eligibility criteria is met and places are available.
