Access HC and Inspiro have merged. The following services are offered: Dietetics, Exercise Physiology, Physiotherapy, Podiatry and Occupational Therapy. OT, Podiatry and Physio are currently experiencing high demand due to staffing. These services are restricted to clients meeting clinical urgency criteria (or semi urgent criteria for OT). For podiatry, clients require a history of foot wounds, or current bone fracture. For Physio/OT, please ensure referrals clearly outline: Diagnosis, information regarding any terminal/ palliative or progressive conditions • Risks associated with pressure care • Falls – where they occurred, the cause, injuries sustained, assistance and medical input required • Safety and independence with transfers and mobility • Cognition • Existing supports – who they live with, carer information, the sustainability of these arrangements, risks to carers • Any risks associated with capacity to remain at home All referrals require a current support plan
