This provider offers services through the Commonwealth Home Support Program.
What are Home Modifications? Changes to a person home to increase or maintain their level of independence, safety, and accessibility so that they can continue to live and move safely about the house. Some examples are handheld shower hoses, handrails, and step modifications. Who is eligible for a Home Modification? Uniting AgeWell is funded via the Commonwealth Home Support Programme (CHSP) as such you will need to have a Home Modification referral from My Aged Care. In addition Home Modifications must be clinically justified and must be linked to improving safety, accessibility and independence within your home environment. How much will it cost? As with all CHSP funded programs customers are expected to contribute. At Uniting AgeWell we charge a subsided labour fee and the cost price of materials.
