Bonalbo Multi Purpose Service
This provider offers services through the Commonwealth Home Support Program.
Community Nursing Service provide and coordinate the delivery of accountable and responsible client centred care using an evidence based framework. Clinical services provided include; • Acute and chronic wound management, • Management of PICC lines and other venous access devices, • Management of catheter and stoma care, • Ongoing assessment, planning and evaluation of client care including relevant and timely referrals to other services, • Supporting Hospital with early discharge and avoidable hospitalisation. Other clinical activities provided include; • The provision of end of life care and support to palliative care clients and their families/carers including pain management and basic nursing support, • Hospital in the Home, • The provision of chronic disease management activities such as, care coordination through the provision of self-management strategies, discharge planning from the inpatient services and ongoing case review.
