Crestwood Aged Day Service provides services to residents of Blacktown Local Government Area, who are older, have an age related illness or are frail. The service provides client centred multidisciplinary health care services to older persons with a chronic, complex and/or aged related illness or frailty.Services include:Individual health assessments;Health and well-being group; activity programs;Transport;Access to nursing and allied health services.Activities may include:Physical activity for balance and strength;Falls prevention;Music appreciation;Arts and craft;Outings;Health information sessions;Mentally stimulating games.Morning tea and lunch is provided.Centre program hours are:Monday-Friday: 10.00am-2.30pm
