This provider offers services through the Commonwealth Home Support Program.
Case Management short term specialised support is available to assist people, their families and/or carers living at home to build their capacity and independence.
Eligibility: People aged 65+ years, or 50+ years for Aboriginal and/or Torres Strait Islander people, people experiencing homelessness on a low or no income and early frail aged.
The team can assist people by providing a holistic approach, targeted support and tailored guidance to improve access to aged care services.
Case Managers use a strength based person centred approach by offering outreach support, referrals to services, advocacy, care co-ordination and assistance to participate in meaningful community activities.
