Planned Activity Groups Merri Outreach Support Service
This provider offers services through the Commonwealth Home Support Program.
Planned Activity Groups provide the opportunity for people who are isolated due to disability or age related issues to come together in a social environment and participate in activities supporting their ability to remain living at home in their communities. A nutritious meal is provided as part of each session. At MOSS all our Planned Activity Groups are located on Department Older Persons estates in spaces shared by residents, who also are welcome for a drop in service. Our aim is to offer physical activities that contribute to the health and well-being of clients, art & craft activities which encourage creativity as well as thinking activities that challenge clients cognitively in an environment in which participation is encouraged. We offer outings and engagement in community celebrations, keeping people connected to their local communities. Our spaces are welcoming and accepting of all individuals.
