South Gippsland Hospital District Nursing
This provider offers services through the Commonwealth Home Support Program.
District Nurses conduct home visits for monitoring health, assistance with administering medication and support. Care includes assessment and monitoring, wound care, catheter care, palliative care, medication management, referrals to other services and care co-ordination. The District Nursing Service provides Home Nursing for clients needing follow-up care after discharge from hospital. Hospital in the Home (HITH) and Post-Acute Care (PAC) Programs are available via service plan agreements with discharging hospitals. District Nursing can also service DVA and TAC clients and can be contracted through Aged Care Packages to provide home nursing. Due to different funding sources please inquire about costs to clients on admission to District Nursing Service.
