Our team of skilled and experienced nurses have a strong commitment to working with clients and their carers in a respectful and personalised way. The team provides comprehensive clinical assessments, generally in the home, and together with client and their family/carers develop a plan on how their identified needs can be met. Our nursing team works closely with the local GPs and other providers and can assist with clinical related care including wound care, medication management, incontinence, lymphoedema, as well as provide support with management of chronic health conditions ( diabetes, Parkinsons diseases, life limiting illness, cancer related care needs and navigating the health care system including referrals and advocacy.
