• Have you had a recent loss? Loss of a loved one, loss of independence or a recent diagnosis?
• Do you feel like you have control over your life? Do you feel taken advantage of by family/friends or other people who support you?
• Have you stopped doing activities that you once enjoyed?
• Are you confused about dealing with My Aged Care, Centrelink, NSW Housing or any other agencies?
If you answer yes to any of these questions our Social Workers within the Allied Health Reablement Team can assist.
We will work with you and your carer/family to address your concerns.
