St Johns Community Care is committed to caring and enhancing consumer’s independence and quality of life. St John's Domestic Assistance Service assists you with a variety of household tasks. We provide flexible, responsive and person centred services valuing people with special needs, appreciating diversity. We will match the consumer and care staff and provide a culturally appropriate staff to support you. We work with you to create a Care plan that’s right for you, maintaining your wellness. Each individuals care plan & services are delivered, monitored, reviewed and reassessed regularly. The Planner develops an understanding of the consumer’s needs, the goals that the consumer has, and a plan with actions that will achieve these goals. St Johns staff undergo training for their role, ensuring quality care services are delivered. St John’s has over 25 years’ experience in aged care, we collaborate with stakeholders in developing care plans/services to achieve person’s wishes.

Getting all your services from one provider
Under the new Aged Care Act, you'll need to access all your Support at Home services from a single provider. When choosing a provider, it's important to consider whether it offers all of the services you need.

St John's Community Care North Brisbane offers home care services through both the Support at Home program and the Commonwealth Home Support Program (CHSP) . You will only be eligible for services under one program—your assessment outcome will outline which program you have been approved for.
