cohealth Senior Connections: Social Support & Respite
This provider offers services through the Commonwealth Home Support Program.
We offer social support groups five days a week across various locations in Carlton, Glenroy, East Keilor, Niddrie and Strathmore Heights. Senior Connections offers opportunities for older adults who may require additional supports to spend time with others making positive social connections. Some groups offer transport within allocated catchment area and groups offer morning tea/lunch, and light movement activities. Our activities include regular excursions, arts and crafts, story sharing and social time with peers and staff. Our staff are trained aged care support workers from diverse backgrounds and experiences. We offer in language support and interpreters at each of our sessions to ensure participants can fully engage. Our social groups are developed and reviewed based on community need and can be targeted for specific cultural groups, general or high needs. All participants receive and individual goal directed care plans which get reviewed annually or as needed
