Review by a Dietitian for nutritional needs, considering individual health conditions such as dementia, as well as a person’s living situation and level of activity. This will include education to improve food and nutrition knowledge, strategies to improve appetite and nutritional intake. Priorities given to those who • have lost weight unintentionally • have had poor appetite and/or reduced intake • are on texture modified diet and thickened fluids • struggle to eat and /or prepare meals
