IRT Assistance with Care and Housing
This provider offers services through the Commonwealth Home Support Program.
Through the Program, the Coordinator helps seniors link to suitable community services, working with them to find sustainable and affordable housing. This support includes: • locating suitable and affordable long-term housing • providing advice and assistance on housing applications • helping people to access accommodation related legal and financial services • assistance with packing, moving and relocating seniors into their new home • accessing relevant local community support to help improve quality of life • where suitable, advocating and negotiating with services • linking and referring seniors, where appropriate, to aged care, community care, healthcare and welfare services, disability services. Those who can access the IRT ACHA Program include seniors who are: • living on low incomes • living in unsuitable accommodation (for example, expensive housing or accommodation which restricts their access to support services) • homeless, or at risk of becoming homeless
