Eyres House program provides weekday social support activities for adults in the community who are living with dementia and memory conditions. Monday to Friday groups for up to 15 participants. On-site activities and outings to local community venues. Limited small bus transport available to assist access to the program. Fee schedule available on request. Currently $20.50 per day for attendance and meal for CHSP eligible participants. Those in receipt of funding through Support at Home and NDIS may participate at an alternative fee schedule if eligibility criteria is met and places are available.
