However, this program will continue under different funding arrangements, with changes to program eligibility, including: • Accessibility across all of Australia • No restrictions for people who have other funding packages in place • Availability to a wider range of ages • No client contributions Referrals can be made online: https://www.dementia.org.au/professionals/gps-and-other-referrers/refer-someone-your-care#refer-someone-today Alternatively, your client can access our individualised dementia support services, by contacting the National Dementia Helpline on 1800 100 500. Dementia Australia’s individualised dementia support includes a mix of direct services, tailored support and expert advice. These services are for people living with dementia, mild cognitive impairment, or individuals seeking a diagnosis (and their carers).
