COMMUNITY OPTIONS ASSESSMENT & CASE MANAGEMENT SERVICES-TASMANIAN HEALTH SERVICE. Community Options provides case management support to vulnerable clients and their carers who have complex support needs, and who are unable to have their needs met through other specialised agencies. These clients are often at risk of repeated hospitalisations and premature or inappropriate placement within residential care facilities.The Community Options case management process encompasses a comprehensive assessment, identification of risk, collaborative care planning, resource identification and the coordination of services to assist a client to attain their identified goals.The overarching aim is to empower the client, increase independence and ensure that clients are involved in all aspects of their own care.
