Our team is here to support & work with customers & their representative/s, in order to prevent disease, maintain health & care for any existing health problems, in the comfort of the customers own home. We aim to support and maintain customer independence, goal achievement, safety, wellbeing & a healthy lifestyle. Our team also ensures support & advocacy on our customers behalf, ensuring the best possible health & wellbeing outcomes. Our team can support with: General health assessment - education, monitoring and support, Chronic Health monitoring, management, education & support, Personal Care – showering, dressing & grooming, Medication management, Diabetic management, Wound management, Continence management/Catheter management, Cognition assessments, Stoma care, Pain management and Palliative Care.
