This provider offers services through the Commonwealth Home Support Program and Support at Home program.

Based on an individual assessment, allied health professionals work with you to establish a tailored treatment plan that can include individual or group therapy. Services may be provided on a short term or episodic basis and are provided to support your independence with a preventative, maintenance, or restorative focus. Services may include: • Counselling • Occupational therapy • Assessment for home safety and equipment needs • Education in use of equipment that supports independence • Exercise/Gym classes/Falls prevention • Tai Chi, Chi Ball • Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) self-management. Sessions are conducted at Murray Bridge and Port Elliot with some services provided in your home (eg Occupational Therapy). Transport to and from venue may be provided, depending on your circumstances.
