Holstep Health - Memory and Dementia Service

The Memory and Dementia Service at HolstepHealth provides assessment and intervention to clients with memory loss and dementia across all stages of dementia. Interventions are offered at the client’s home (for clients in the northern metro region of Melbourne - see postcode list) or at the clinic at Glenroy. Services offered include: Occupational therapy (assessment & interventions for changed behaviours, home based environmental enhancement & design, memory strategies, activity planning, sensory modulation intervention, carer education regarding communication, home modifications & equipment prescription), Physiotherapy (gait aid prescription, mobility/balance assessments) exercise physiology (individualised exercise programs), speech pathology (communication assessment & intervention) and dietetics. Geriatrician and carer support/service coordination services are also available. Services are not time limited and clients can be re-referred to the service as required.
