Holstep Health - Rapid Reablement (MERRI-BEK)
This provider offers services through the Commonwealth Home Support Program.
Intensive goal-based therapy delivered over 6-8 weeks following an acute health event. Goals are functional in nature. Appropriate clients identified by the Regional Assessment Service (RAS). Full allied health services are offered to clients either as a home-based or centre based service at Glenroy, Coburg, Fawkner and Brunswick. Home visits are offered to consumers residing in the Northern metropolitan region of Melbourne
