This provider offers services through the Commonwealth Home Support Program and Support at Home program.

Where family or other support is not available, Moyne Shire Community Care offers transport assistance to local appointments or scheduled activities. Our transport service includes local or short transport (under 15 minutes one way) and long transport (up to 30 minutes one way). Where accompanied support is requested for appointments, our Community Care Workers can deliver this service under our Social Support Individual program. Transport and Social Support services include a client contribution fee. All transport bookings are subject to availability of Community Care Workers.
