This provider offers services through the Commonwealth Home Support Program.
Home modifications can be made to eligible resident’s homes once they have been assessed by Occupational Therapists. Home Modifications suggested by OT's aim to increase your functional independence so that you can continue to live and move safely around your own home. Examples can include installation of grab rails, threshold ramps, step modifications, bed and chair raisers, door handles, hand-held showers and safety hinges, as well as installations and fitting of emergency alarms and safety aids.
