Community Dementia Service North
This provider offers services through the Commonwealth Home Support Program.
Community Dementia Service (CDS) provides consultation, assessment, information, and the provision of direct care to people living with a diagnosis of dementia in the Greater Launceston area. The service works in partnership with clients' families and other healthcare professionals. • Provides in home assistance with all medication management including insulin and scheduled medications • Assists with hygiene and nutrition support for individuals experiencing challenges in areas of independence • Behavioural and case management specialists in dementia, continued client and carer support • Providers of all their Nursing care including-continence, wounds, and health support • A Clinical Nurse Consultant can conduct Cognitive Assessments via GP referral for individuals requiring an assessment to aid them in determining the diagnosis of Dementia, in Northern Tasmania. The Community Dementia Service also operates a dementia specific Day Centre, Mayne Street Day Centre in Invermay.
