Our Chronic Disease Management Programs provide safe and confidential services for individuals living with chronic and complex conditions. Our nurse led programs are staffed by experienced chronic disease registered nurses, diabetes nurse educators and health coaches who support participants to self-manage their condition(s) and stay well at home through education and service linkage.
