Home Based Dietetics
This provider offers services through the Commonwealth Home Support Program.
Sydney Local Health District offers a community based Dietetics Service that restores, improves or maintains a frail, older people’s health, wellbeing and independence. To be eligible they must be a frail, older person who:
• Is aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander peoples)
• Lives in the community and cannot access outpatient services (there are a range of outpatient or specialist dietetics services that may be more suitable).
