NT Transition Care Program is a short-term restorative program providing support to frail aged clients following hospital stay. Services/supports provided in clients own home including personal support and therapy as required.
One Residential TCP place is available in Alice Springs, these also managed from Darwin Unit in conjunction with staff on ground at Alice Springs.
