Continence Nursing
This provider offers services through the Commonwealth Home Support Program.
Sydney Local Health District offers Continence Nursing interventions (assessment and management) to people living in the community, for whom the home setting has been identified as the most appropriate place for the therapy. The person must be within the CHSP target group ie. a frail, older person who:
• Is aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander peoples)
• Lives in the community.
People able to travel may be referred to a clinic setting at either Concord or Balmain Hospitals.
