This provider offers services through the Commonwealth Home Support Program.
The Community Nursing Service will provide:-
Ongoing assessment, planning and evaluation of client care including making relevant and timely referrals to other services;
Management of PICC lines and other venous access devices; chronic wound management;
Other clinical activities such as catheter and stoma care;
The provision of chronic disease management activities such as preparing a management plan, care coordination through discharge planning from the inpatient services and review through the case review process and the provision of self-management strategies and
Assessment and appropriate referral of clients and carer's physical, psychosocial, emotional and environmental health needs.
Supporting Hospital with early discharge and avoidable hospitalisation.
