Access and Support Hume City Council
This provider offers services through the Commonwealth Home Support Program.
Access and Support works with people who are eligible for the Commonwealth Home Support Programme to access aged care services to improve their wellbeing and ability to live in the community as independently as possible. The Access and Support Officer provides short-term episodic support to people: - From the Aboriginal and/or Torres Strait Islander community - From culturally and linguistically diverse (CALD) backgrounds - Experiencing financial or social disadvantage - Veterans - Experiencing homelessness, or at risk of homelessness - From the lesbian, gay, bisexual, intersex or transgender communities - Care leavers - Parents who have been separated from their children by forced adoption or removal Please note that case management is not available under this program.
