Sydney Local Health District offers Occupational Therapy in the home that restores, improves or maintains frail, older people’s health, wellbeing and independence.
Those eligible are a frail, older person who:
• Is aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander peoples)
• Lives in the community and cannot access outpatient services
• Receiving CHSP service prior to admission to hospital.
They require either therapy (of an ongoing or episodic nature), at a low intensity or frequency, with a maintenance or preventative focus; or restorative care for clients who can make a functional gain after a setback with the goal to increase their independence.
