Uniting War Memorial Hospital Outpatient Services
This provider offers services through the Commonwealth Home Support Program.
War Memorial Hospital offers a Patient Transport Service for those patients/clients who need to attend outpatient clinics. The patient or client is able to discuss and request hospital transport with their treating clinician. The clinician will submit a transport request on the patient's behalf.
Eligibility Criteria:
1. My Aged Care CHSP Transport Code
2. Patient/client must live within the Hospital catchment area, i.e. Waverley LGA, Randwick LGA and parts of Woollahra LGA. A detailed list of suburbs is available.
3. Patient must be able to comply and adhere to bus safety rules, such as wearing a seat belt, stay seated while in transit and follow the directions from transport staff.
4. Transport buses must be able to safely access the patient's area of residency to provide a safe working environment for the patient and staff when boarding and alighting from the bus.
5. Patient must pay a transport fee. Payments are processed on arrival to the hospital at reception.
