Uniting War Memorial Hospital Outpatient Services
This provider offers services through the Commonwealth Home Support Program.
Services are provided as part of the iREAP Day Rehabilitation program based at the hospital. No individual Occupational Therapy, Physiotherapy, or Dietetics is available to clients outside this program. Uniting War Memorial Hospital’s integrated Rehabilitation and EnAblement Programme, ‘iREAP’ provides comprehensive early intervention care to clients at risk of frailty and falls, or those with a degenerative neurological disease such as Parkinson’s. Each client receives an individualised multidisciplinary programme of care, tailored to their particular needs. In addition to Physiotherapy, Occupational Therapy, and Dietetics clients can access Speech Pathology, Podiatry, Social Work, Continence Nursing, Psychology, and Geriatric Medicine assessment and treatment according to their clinical need as part of the programme. Clients must be able to commit to attend the program for 3 half days per week, for 8 weeks.
