General Criteria The person: • Is an older adult living in the community (i.e. not residing in residential aged care) • Requires specialist mental health assessment, treatment, or support Clinical and Psychosocial Criteria The person meets one or more of the following: • Is socially isolated or has limited social supports • Lives alone, or lives with a partner who is frail, aged, or has a disability • Has a diagnosed or suspected mental illness • Is experiencing deteriorating physical health impacting mental wellbeing • Is vulnerable to further mental, physical, or psychosocial deterioration • Is experiencing Behavioural and Psychological Symptoms of Dementia (BPSD)
