This provider offers services through the Commonwealth Home Support Program and Support at Home program.
After a fall, illness, injury or change in your health, you may need some help to get back on your feet and live safely and independently. Our expert multidisciplinary care teams have helped countless Australians improve their mobility and independence, all from the comfort of their own homes. We start by understanding your needs, strengths, and areas that you’d like to improve. We also help you identify any risks to your safety. After your assessment, our team will provide you with advice, strategies and support to improve your wellbeing. Our occupational therapy services can help you live safely and independently at home. This can include home modifications, mobility equipment, and pressure management techniques and by reducing the risk of you falling at home. Our physiotherapy services can help maximise your strength, balance, and mobility. We do this through targeted exercises, activities, movement techniques, and practical support.
Getting all your services from one provider
Under the new Aged Care Act, you'll need to access all your Support at Home services from a single provider. When choosing a provider, it's important to consider whether it offers all of the services you need.

Silverchain offers home care services through both the Support at Home program and the Commonwealth Home Support Program (CHSP) . You will only be eligible for services under one program—your assessment outcome will outline which program you have been approved for.
