Exceptional Needs Unit - Homelessness Support Program
This provider offers services through the Commonwealth Home Support Program.
To be eligible for the Homelessness Support Program a person must be homeless or at risk of homelessness and aged 50 years or older The Program prioritises its focus on people (aged 50-64) who have a history of chronic homelessness and exhibit characteristics of premature ageing. This may include people entering or exiting the Supported Residential Facilities (SRF) sector. This is a flexible support service, which is time-limited (up to 3 months) and aims to assist a person to achieve lifestyle stabilisation and independence. The goal is for the person to gain the skills and confidence to be independent of support services or linked to a lead agency at the conclusion of the support service contract. To access services a person must meet the ACHA criteria and be eligible for services from the Exceptional Needs Unit (ENU)
