This provider offers services through the Commonwealth Home Support Program and Support at Home program.
Jubilee Community Care is a not for profit aged care service provider, established in 1989. We are a local service provider to your area with our offices based in Indooroopilly. We provide care and support services enabling people to enrich their life by providing personalised services. We will organise your care and support services on the day(s) and time(s) of your choosing and with regular Support Workers. Our Support Workers are employed on permanent contracts of employment, reference checked, have a Police Clearance Certificate, attend annual and on-going training including First Aid and CPR, and are supervised by experienced Client Support Coordinators. Clients of Jubilee Community Care can access hydrotherapy and exercise classes, falls prevention programs and weekly social activities and events connecting you with your community.

Jubilee Community Care offers home care services through both the Support at Home program and the Commonwealth Home Support Program (CHSP) . You will only be eligible for services under one program—your assessment outcome will outline which program you have been approved for.
