This provider offers services through the Commonwealth Home Support Program.
This service is provided by Community Nurses either in their homes or Community Nursing Clinic.
Persons with a chronic disease requiring care,management and integration of services
Persons requiring wound management related to chronic disease, complex wounds
Persons requiring continence assessment and management related to chronic disease
Persons requiring palliative and supportive care at the end of life
Persons requiring a Nursing procedure who are mobile and have access to transport will attend the local Community Nursing Clinic
All clients referred to the Community Nursing service receive a comprehensive assessment. Clients are case managed through care planning, coordination and case review.
Education and support of the client and/or their carer is also provided.
Client’s physical equipment needs are assessed and appropriate referrals or arrangements coordinated. Referrals to other service providers are made based on the assessment and client needs.
