Based on an individual assessment, allied health professionals work with you to establish a tailored treatment plan that can include individual or group therapy. Services may be provided on a short term or episodic basis and are provided to support your independence with a preventative, maintenance, or restorative focus. Services may include: • Podiatry • Occupational therapy • Physiotherapy • Formal counselling • Assessment for home safety and equipment needs • Education in use of equipment that supports independence • Exercise/Gym classes/Falls prevention • Tai Chi, Chi Ball • Condition-specific (Parkinson’s, stroke/pulmonary conditions, arthritis, diabetes) self-management. Sessions are conducted at Elizabeth with some services provided in your home (eg Occupational Therapy). Transport to and from venue may be provided, depending on your circumstances.
