Merriwa Community Health Service - Nursing - General
This provider offers services through the Commonwealth Home Support Program.
Service provided by Community Nurses, in the Client/carers home or Community Health Centre Nursing Clinic. Services include: wound care, chronic disease management and general nursing procedures. All clients referred to this community nursing service will receive a comprehensive assessment and are case managed through care planning, coordination and regular reviews. Education and support is given to the clients and or their carer’s and referrals to other service providers may be completed based on the assessment and clients individual needs.
