This provider offers services through the Commonwealth Home Support Program.
The Connections program provides outreach to people who are homeless or at risk of homelessness and who have complex unmet health needs. Work typically occurs on public housing estates, rooming houses and other low cost accommodation settings. Criteria is singles and couples on low incomes who are experiencing housing issues and have complex health and or social needs.
