St John's Community Care Innisfail
This provider offers services through the Commonwealth Home Support Program and Support at Home program.
St John’s In-home respite assist the person who has a carer. We support the person with the things that the carer usually helps with and gives the carer a break from the caring role, it may include personal care and meal preparation, as well as social activities either in the home or the community. We provide flexible, responsive and person centred services valuing people with special needs, appreciating diversity . We will match the consumer and care staff and provide a culturally appropriate staff to support you. We work with you to create a Care plan that’s right for you, maintaining your wellness. Each individuals care plan & services are delivered, monitored, reviewed and reassessed regularly. St Johns staff undergo training for their role, ensuring quality care services are delivered. With over 25 years’ experience in aged care, we collaborate with individuals, families, health services, community groups and agencies in developinig care plans to achieve a person's wishes.
