- People living with diabetes and complex clinical, social and/or behavioural needs at risk of potentially preventable hospitalisations
Services provided:
- Comprehensive holistic assessment to identify client health and wellbeing needs
- Remote monitoring of medical conditions
- Health education and interventions to improve client’s ability to self-manage
- Goal setting
- Linkages to required support services to achieve health and wellbeing goals
- Social Prescribing
- Service navigation and advocacy for aged care services to support client to remain living at home
