Rural health service that follows the Wellness & Reablement approach. We offer the following services Chronic disease management (e.g. diabetes) Health education and advocacy (e.g. care plans) Medication management (e.g. monitor usage) Observations and assessment (e.g. blood pressure) Pain and symptom management (e.g. nausea) Palliative care (in hospital and at home) Wound care (e.g. changing dressings) We also have specialsed nursing services which are delivered by our Continence Nurse Consultant and Diabetes Educators. Central Highlands Rural Health is committed to providing inclusive care to LGBTIQA+ people and is Rainbow tick accreditated.
