JCS Home & Community Support Services (CHSP)
This provider offers services through the Commonwealth Home Support Program.
JCS provides Social Support services to help older people stay connected, reduce social isolation, and maintain wellbeing while living at home.
Group Social Support offers opportunities to connect with others through group activities, outings, and programs that encourage participation, friendships, and a sense of belonging.
Individual Social Support provides one‑to‑one support for people who prefer or require individual assistance, focusing on companionship, confidence‑building, and participation in meaningful activities at home or in the community.
All social support services are person‑centred and responsive to individual needs, with a strong focus on our “Active Choices, Active People” model. Wherever possible, we support choice, independence, and active participation.
